چکیده

IT IS WELL KNOWN that in polyeythemia rubra vera the total blood volume increases as the hematocrit level rises beyond approximately 55 per cent.1-2 This increase masks the extent of the polycythemia and insures that the plasma volume is not greatly diminished despite an increase in red cell volume to twice or three times the normal value. The changes in adults suffering from cyanotic congenital heart disease have not been precisely defined. It is less easy to compare the blood volume in this condition with expected normal values because the limits of the normal blood volume are poorly defined by reference to height or weight.3 Statistically valid limits of blood volume have been determined in adult subjects by means of height, weight, girth, and fat thickness.3 In this paper patients of adult stature suffering from polycythemia rubra vera and from congenital heart disease are compared with normal adults.

Gout in Cyanotic Congenital Heart Disease.

Gout complicating cyanotic congenital heart disease is infrequently recognized. Its occurrence in blood dyscrasias associated with hyperuricamia is well documented (Gliickmann, 1910; Weber, 1934; Barr et al., 1950; Hickling, 1953). In polycythemia rubra vera, gout occurs in 5 to 9 per cent of patients (Tinney et al., 1945; Videbaek, 1950; Lawrence, 1955) and the complication has been referred t...

Editorial Management of polycythaemia in adults with cyanotic congenital heart disease Adults with polycythaemia secondary to cyanotic congeni-

Adults with polycythaemia secondary to cyanotic congenital heart disease may be at greater risk from injudicious venesection than from their polycythaemia. Despite an established literature on the subject, patients are frequently put at risk from acute volume depletion and chronic iron deficiency. This article aims to clarify some of the issues surrounding venesection and to set out guidelines ...

Acute hemodynamic effects of red cell volume reduction in polycythemia of cyanotic congenital heart disease.

Acute reduction in red cell volume (RCV) without significant alterations of blood volume in 22 patients with severe polycythemia secondary to cyanotic congenital heart disease resulted in a decrease in peripheral vascular resistance and an increase in stroke volume, systemic blood flow (SBF), and systemic oxygen transport. These changes are probably related to the decreased blood viscosity and ...

Haemodynamic effects of haematocrit reduction in patients with polycythaemia secondary to cyanotic congenital heart disease.

Acute reduction in haematocrit (without significant alteration of blood volume) in six patients with severe polycythaemia secondary to cyanotic congenital heart disease resulted in an increase in resting cardiac output without alteration in heart rate. Cardiac output was also increased during a constant-load exercise test after haematocrit reduction, and under these conditions total oxygen upta...
